Final Proposal
There are no application forms when submitting the final proposal though the Common Grant Application Form may be used. Applicants are asked to submit one copy of the following information:
a) Cover Sheet with summary information: name, address, phone number, contact person, size of grant requested, brief synopsis of project, and brief summary of organization’s past accomplishments.
b) Narrative describing the organization, the issue being addressed, specific strategies used, including a detailed workplan, the qualifications of the people carrying out the plan, and the evaluation process to be used to determine its effectiveness.
c) Organization budget and separate project budget if applicable (line-item).
d) Sources of other funding, received and pending.
e) IRS 501(c)(3) notification letter (if not previously submitted) of the organization or its fiscal agent.
f) Board member list, if applicable.
